SWLSTG and Riverside have a contract to provide intensive rehabilitation support in the community for 23 Wandsworth clients across two houses, Thrale Road and Westmoor House.

The services provide social housing support for people with complex mental health needs currently under the care of SWLSTG Mental Health NHS Trust for up to two years. The service delivers a high quality, recovery focused supported housing service which facilitates earlier discharge from hospital and reduces reliance on residential care placements. Clinical input from SWLSTG consists of this occupational therapy post, clinical psychologist, consultant psychiatrist and a full time dedicated community psychiatric nurse as the care coordinator across both houses.

In this role you will provide specialist OT assessment and interventions for the clients at Thrale Road and Westmoor House. In most cases, support staff will take the lead in pursuing OT recommendations, but where interventions require, there is scope for longer term 1:1 and groupwork.

· To manage a caseload of clients with complex needs, using evidence based and client centred principles to assess, plan, implement and evaluate interventions.

The role of the specialist screening practitioner (SSP) is to follow the patient through their cancer pathway. The SSP will assess patients for their suitability to go on to have diagnostic tests, follow up histology results and inform patients of their ongoing care pathway if appropriate.

The North East London Screening Centre (NELSC) is comprised of three sites: Homerton healthcare NHS Trust, Whipps Cross Hospital and the Royal london/ Mile End Hospital. The newly appointed SSP may be required to work at any one of these sites although once the training period has been completed, you will be based at one site for the majority of the time. We are a small, friendly team who offer support and guidance to each other when required.

The post holder is responsible,as part of a multidisciplinary team, for delivering a comprehensive bowel cancer screening service. The postholder will be responsible for care delivery for patients who have been identified by the Bowel Cancer Screening Programme who require follow up care. They will be expected to provide and co-ordinate care from the screening centre to address the needs of the individuals identified by the Screening Programme. There will be a period of training and induction for the postholder to be able to competently carry out this role.



To assess whether a patient is suitable to undergo a diagnostic test via the screening programme. To counsel the patient and allay their concerns and fear regarding the diagnostic test. The SSP acts as an advocate for patients within bowel cancer screening. The SSP follows up histology and scan results and together with the screening endoscopist, plans the appropriate pathway for the patient. SSP's work closely with the Colorectal MDT's as patients who are diagnosed with cancer are monitored closely to ensure correct pathways and targets and standards are met.

Roles and responsibilities
• Provide a customer focused service dealing with all patients or visitors in an efficient, polite and courteous manner
• Support the delivery of a high quality healthcare service ensuring that all patients receive a high standard of service delivery
• Handle queries from patients/relatives confidentially and sensitively; refer clinical related queries to appropriate staff
• Interact with patients and relatives in distress in a sensitive and empathetic manner
• Take and relay as appropriate, telephone enquiries and messages , using initiative and prioritising skills when disseminating information
• Update patient details on PAS as appropriate, checking all demographic details on arrival or departure
• Oversee the work of more junior staff and ensure work objectives are being met on a day-to-day basis and demonstrate duties to new starters
• Ensure all procedures are followed re data capture for service specific outcomes.

• Arrange interpreter and transport services where indicated

Surgery across the Royal London Hospital has been redesigned to ensure we provide excellent care for all our patients. Ward 10E has been instrumental in tackling the challenges ahead and reducing our waiting times for urgent surgical care. Wards 10E manage the following specialties:

Surgical Admission ward: The ward works closely with our emergency department. Our main aim is to create a smooth admission pathway from the front door to go through either our emergency surgical ambulatory care unit or onto our admissions ward.

Vascular Care Unit: Dedicated beds to the management and care of our vascular patients. This will involve close MDT working for patients requiring urgent and emergent vascular procedures.

Nurses working on these wards will develop expert knowledge in assessment, complex wound management, care of cancer patients, management of vascular conditions and effective patient flow.

We are looking for enthusiastic nurses with a variety of experience who will use there initiative to improve service delivery. We need nurses who are willing to adapt and work with the vascular team to ensure we embed these changes for the benefit of our patients. Our practice development nurses and vascular nurse specialists provide day to day support and have established an education programme for all your training needs.
