An exciting opportunity has arisen to appoint a Deputy Manager to join our team supporting 5 adults with learning disabilities at Sackville Gardens, Hove.

We are looking for someone who has excellent support skills with people who have a learning disability, combined with experience of the supervision of staff.

As part of the leadership team we would like you to have a strong understanding of legislation and best practice and the role will include supporting the Service Manager to lead the team in providing high quality support for the clients that live at Sackville Gardens.

The purpose of a Home Care Packages Case Manager will be to provide support, maintain accurate documentation and coordinate internal and external home care services for clients who have been allocated Home Care Packages.

Case Management

Support and facilitate the provision of the allocated Consumer Directed Care packages via the accurate completion and lodgement of documents.
Develop and document a strengths based care plan with the client and their carer/representative to implement services which will met their individual needs.
Provide regular review meetings according to policy and procedures, which involve the care recipient, family, carers and community service providers including the client’s GP and other appropriate personnel, making modifications to the care plan as required.
Ensuring that accurate information and advice is provided to residents, service users, staff, the Coordinator, Allied Health and In Home Support, Director, Community Services and other agencies
Support the ongoing access to Consumer Directed Care by working, in consultation with the consumer, the establishment of a budget and associated care plan.
Provide ongoing case management for up to 60 Home Care Packages (HCP).
Monthly budgets are finalised and made available to all recipients of a Commonwealth funded Home Care Package.
Generation of HCP monthly activity reports via the client management system (Etools/Carelink+).
Maintain working knowledge and practice of Aged Care Commission Standards in all aspects of case management..
Liaise with the Service Coordination team to provide consumers with client focussed care, including accurate information of consumer needs for service delivery.
Receive new client applications through internal or external referrals.
Contact new clients to arrange an appropriate and convenient time to visit, and provide the client with the appropriate, relevant and applicable documents in accordance with company policy, including client agreements, care plans and fire safety plans, and ensuring all documents have been thoroughly completed.

The Secure Extended Care Unit (Unit 4) is a 50 bed Inpatient Unit based at Dandenong Hospital. SECUs provide medium to long-term inpatient treatment and rehabilitation for people who have unremitting and severe symptoms of mental illness or disorder with an associated significant disturbance in behaviour that preclude their living in a less restricted environment. SECU has a recovery-based model of care. Experience working within the Construction industry, specifically within a general Civil Engineering environment
Previous experience of infrastructure, drainage, public realm, highways and structures
Proven track record with delivery of large-sized schemes or supervisory experience of work on multiple schemes at any one time

Practical hands-on approach
CSCS black card
SMSTS qualification
Lift supervisor
First aid qualification

Clinical Nurse Lead / Deputy Manager

Woodside Hall Nursing Home is a 41 bed purpose built home in a beautiful setting on the Isle of Wight. The Home has a CQC rating of "Good" with "outstanding" for "Care".

This role is a clinical role with leadership responsibilities. You will take charge of the Home on shift and also provide on-call cover as part of a rota. You will be responsible for all aspects of the Home's provision of Nursing care, with the support of the Registered Manager. The successful candidate will be confident in their own clinical skills and be able to prioritise and delegate to other members of the Nursing team and Senior Carers. The role will also include induction and supervision of Registered Nurses and Senior HCA’s.

This role will suit a confident Registered Nurse with prior experience in a leadership role or a nurse who is looking for career progression in a clinical role. They will be passionate about palliative care. We will support with maintaining and further development of clinical and leadership skills.

The successful applicant may have nursing home experience or have experience of acute settings, but will enjoy sharing their skills with others and support management with identifying the training needs of our Registered Nurses.

You will be computer literate and confident with the use of EMAR and Care Management software. You will be able to write effective care plans, complete mental capacity assessments, CQC notifications and Safeguarding referrals. You will be able to demonstrate competence in medication management and take the lead for medication management in the Home.

This is a full time position, 36 or 42 hours a week contract available. Paid breaks, excellent overtime rates and weekend supplements. You will be flexible and available to work alternative weekends. You will be required to work occasional night shifts to cover annual leave. Shifts are 7.30 to 13.30, 13.30 to 19.30 and 7.30 to 19.30
