About the Job:

The Social Worker in the foster care team supports children who are placed in alternative out-of-home arrangements (foster care, kinship care or nanny care).

·      Formulate detailed reports on a students’ social and family background, including financial needs and support systems to aid in the school’s holistic student developmental planning and reporting to government agencies (e.g. Child Protective Service(CPS))

·      Consistently connect with families at students’ key transition points (including orientation, progress meetings, transition planning discussions and graduation ceremony) to foster close school-family collaboration

2.     Post-School Transition Support
