Job Purpose

The Case Worker supports individuals and families facing social, emotional, and practical challenges. The role involves case management, assessment, intervention, and collaboration with community partners to promote safety, stability, and well-being.

Key Responsibilities

• Conduct intake assessments and develop appropriate intervention plans

• Provide case management support to individuals and families with complex needs

• Coordinate with internal teams and external agencies (e.g. community partners, hospitals, schools)
