To effectively manage cases receiving foster care services and to support programmes/events of the Fostering Department and the Agency.
A. FUNCTIONAL
· To conduct in-depth assessment and prepare Foster Parent Assessment Report (FAR) and present findings at the MSF Approving Panel and follow up on decisions and recommendations.
· Conduct regular CANs assessment to assess how the foster child is doing in foster care as well as the foster parent’s capability in meeting the child’s needs.
· Conduct all the necessary statutory visits including announced and unannounced home visits to ensure that the placement is able to safeguard and promote child’s safety, stability and wellbeing.
· To organise and participate in inter-agency efforts in support the welfare and development of the foster child/CYP
· To provide a 24-hour hotline for foster parents/kinship care per duty schedule and ensure that the team follows up on outcomes
· To support foster placement to prevent breakdowns in foster child’s existing care arrangements
· To participate actively in case management through regular team meetings, Monthly case review sessions, MSF case reviews, COF, reviews etc.
· To identify at-risk situations and develop strategies and intervention plans to address child safety issues, and wellbeing.
· To monitor and evaluate effectiveness of case management and other support programme and activities extended to foster/kinshi care families.
· To submit accurate reports punctually for operation reports, case reviews, discussions to BT and other governing bodies such as MSF.
· To participate in road shows and other community outreach events as and when necessary.
· To meet regularly with Head BTFS/supervisor to assess case progress and programme outcome/effectiveness.
B. ORGANIZATIONAL
· To participate actively in all fundraising activities as well as any other commitments that Boys’ Town undertakes.
· To work in close collaboration with other staff, departments and components of Boys’ Town.
· The Executive Director and or his/her representative may assign you any other duties that are within reason for the benefit of the clients and/or Boys’ Town.
· Executive functioning skills – planning, coordinating, organising, implementing and evaluating
· Administrative – documentation, filing, report writing
· Social Work and Counseling skills – assessment and case management
· Understanding the impact of fostering/kinship care on children/CYP and able to preempt and address the developmental needs of children/CYP.
Average Qualifications & years of relevant experience
· Bachelor of Social Work / Graduate Diploma in Social Work / Master of Social Work
· Registered Social Worker with SASW preferred
· Experience in working with children/CYP and their families
