JOB DRESCRIPTION
Burial Management

• Handle the administration and filling of the burial (NEA, NOK and volunteers)
• Handle query on burial management
• Replace Dakwah team when needed.
• To coordinate with involved parties (eg. Volunteers, NOK) for the preparation and post ceremony.
• To keep Database of volunteers and NOK

Qurban Local & Overseas

• Handle the administration and filing
• Upkeeping database of participants (Sahibul)
• Replace Dakwah team when needed
• To coordinate with involved parties (Eg. Volunteers, Authority & suppliers) and other relevant departments for the preparation and post ceremony.
• Handles queries on Qurban

Ramdhan Iftar

• To attend to query & donation for iftar
• To coordinate with involved parties (Volunteers, Suppliers etc) and other relevant departments for the preparation and post iftar
• To mobilize AMIL duty and safe keeping of collections

Catering (Friday Prayer & KABA Class)

• To coordinate with involved parties for the preparation
• To mobilized CSS Staff for collection
• Handles the administration, filling & cash collection for Finance

Hajj & Umrah

• Handle Admin
• Handle query on Hajj and Umrah
• Upkeep database of pilgrims

Other duties

• Assist in monthly report
• Annual budget submission
• Answer phone/Email/walk-in queries

REQUIREMENTS

• Minimum GCE 'O' level or equivalent qualification/ relevant experience.
• Able to read and write in Malay and English
• Preferably possess a valid Class 3 Driving License
• Physically fit and able to perform field duties
• Willing to work long hours, weekends and off hours when required
• Computer literacy is an added advantage
• Passionate about community service and ready to contribute actively to welfare programmes

JOB DESCRIPTION

• Manage casework processes from admission to discharge, including assessments, care planning and documentation.
• Complete required assessments on residents and families using appropriate tools to inform intervention plans.
• Assess residents’ needs, strengths and risks; develop and review Individualized Care Plans (ICP).
• Provide counselling and interventions for residents and families, including planning and executing counselling sessions.
• Maintain accurate case records, reports and updates in relevant systems (e.g. Case Central).
• Coordinate with internal teams and external agencies to support residents’ progress and family outcomes.
• Prepare and present reports to relevant authorities (e.g. MSF, Youth Court).
• Participate in case discussions, meetings and supervision sessions.
• Support any additional duties or projects as assigned.

REQUIREMENT

• Degree in related field
• 1–2 years of relevant experience in casework and counselling.
• Strong interest in working with children, youths-at-risk and family.

Job Title: Staff Nurse/Senior Staff Nurse

Location: Muhammadiyah Health and Day Care Centre

Clients

1. Comply with approved standards of nursing practices.

2. Perform nursing care such as medication administration and packing, wound dressing and

other clinical skills as required under approved CBN services.

3. Provide appropriate interventions to clinical abnormalities found in client.

4. Liasing with clients external health providers and family for continuity of acute and

chronic health management.

5. Sourcing and directing clients to appropriate external health and medical services.

6. Provide health education to clients and families.

7. Liasing with AIC regarding clients referrals and services.

8. Perform pre-assessment of new clients referred by AIC.

9. Ensure clients enrolment comply with the Company and MOH SOPs.

10. Collaborate with allied health care team to assess, plan, implement, monitor and evaluate

clients progress and care plans.

11. Case management for clients with care plan issues.

12. Assessing the needs and delivery of quality geriatric / dementia care to clients.

13. Assess, monitor and review clients assessments and documentation (e.g. Fall risk

assessment, ZBI survey, NMTS report, Transport triage, ICP, Clients rehab review, FAST score,

Wound Chart, medication chart).

14. Perform CPR, AED and First Aid to clients and plays an active role in emergency situations.

Centre

15. Oversee and manage Maintenance Day Care, Dementia Day Care, and Centre-based

Nursing services.

16. Prepare and assist in audits (MOH for services, NEA for environment and hygiene).

17. Prepare and update centre's nursing SOPs and Guidelines.

18. Replying public and clients enquiries regarding referral process, services and

subsidies available.

19. Oversee centre cleanliness and food supplied for clients.

20. Liasing with external vendors.

21. Arranging medical events at centre or health booth during organisation events.

22. Procurement of medical supplies and equipment.

23. Asset listing of equipment.

24. Arranging maintenance repair duties when necessary.

Staff

25. Oversee and manage the performances of care staff and kitchen helper to ensure

desired level of work competency.

26. Planning of manpower and staff leave.

27. Planning and applying of courses for staff for training and development.

28. Assess and update staff competency skills.

29. Ad-hoc duties assigned by Manager.

Requirements:

• Diploma or Degree in Nursing.

• Valid nursing registration with Singapore Nursing Board (SNB).

• Minimum 2 years’ experience in acute care.

• Strong advantage for candidates with ILTC experiences.

JOB DESCRIPTION

Personal and Psychosocial Care

• Provides support to seniors in basic activities of daily living (BADL), including assistance with personal hygiene and transfer.
• Identify care needs of seniors such as dietic needs and other areas relating to well-being of elders.

Nursing Support

• Attend to basic nursing and general care needs of clients
• Monitor health status (vital signs, glucose level and oxygen saturation) and escalate to Centre Supervisors/ Nurse Manager.
• Perform tube feeding / Nasogastric tube (NGT) feeding for seniors
• Perform basic wound care
• Assist to serve medication/ pre-packed medication and apply prescribed topical medication as instructed by Supervisor/ Manager.

Rehab Care

• Assist Physiotherapist/ Occupational therapist in clients prescribed therapy.
• Assist to manage movement dysfunction and enhance physical and functional abilities.
• Assist to restore, maintain, and promote optimal physical function, optimal wellness and fitness movement.
• Assist in client ambulation movement within the centre and during transferring to and from transport vehicle.

Senior Engagement

• Plan, and conduct activities suitable for seniors taking into consideration of their preferences and interests
• Observe senior’ engagement during sessions and feedback on senior’s response and progress.
• Work with the therapists on implementation of new activities
• Provide support and involve in the planning and coordination of Active Ageing Programmes

Administration and Operations

• Always ensure environmental safety and hygiene of the centre including carrying out the necessary cleaning and disinfection and looking out for hazards
• Able to do basic documentation of senior’s participation in various engagement activities.

REQUIREMENTS

Experiences:

• Experience in ILTC sector is an advantage
• Therapist assistant cert would be an added advantage

Qualifications:

• GCE ‘N’ Level and above

REQUIREMENTS

• Degree in Communications, Marketing, Business or a related discipline
• Minimum5 years of experience at senior management level in corporate communications, public relations, or stakeholder engagement or equivalent.
• Experience in a leadership role, with proven experience managing communications teams.
• Strong strategic thinking and stakeholder management skills, with hands-on experience in media relations, leadership communications, and issues management.
• Excellent written and verbal communication skills, with the ability to engage and influence stakeholders at all levels.
• Strong analytical, planning, and organizational skills, with the ability to manage multiple priorities effectively.
