The Medical Social Worker is to conduct mental health assessment, provide home based direct intervention, support to clients and/or caregivers with mental health issues, and/or dementia in accordance to the Community Intervention Team (COMIT) programme requirement. Responsibilities and Duties:

Job Requirements:

- Degree / postgraduate diploma in Social Work.

- Must have an interest to work with the adults above 40 years old and the elderly population in the community setting.

- Experienced in Community /Hospital setting for at least 2-3 years.

- Good team player and possess good interpersonal, communication, writing & IT skills.

Role Overview

Join a multidisciplinary team (MDT) comprising doctors, nurses, allied health professionals and community volunteers to deliver holistic social work services to End-Stage Renal Disease (ESRD) patients and their families. You will play a key role in supporting patients’ psychosocial well-being, facilitating access to care, and enhancing their quality of life through case management and programme development.

Key Responsibilities

1. Case Admission & Assessment

• Manage the end-to-end referral and admission process, including intake screening, financial counselling, and dialysis slot allocation
• Conduct comprehensive psychosocial assessments and develop individualised care plans for patients and their families
• Collaborate with internal MDT members and external referral partners to ensure smooth care transitions
• Provide orientation and emotional support to newly admitted patients to ease adjustment into the dialysis centre

2. Case Management

• Perform regular patient check-ins during dialysis sessions to monitor well-being and build therapeutic relationships
• Identify and proactively intervene in cases of psychosocial risks, treatment non-adherence, or financial challenges
• Conduct home or hospital visits where required to better assess patient needs and family dynamics
• Develop, implement, and review care plans for complex or high-risk cases
• Advocate for patients and connect them with appropriate community resources and social services

3. Programme Planning & Implementation

• Design and deliver patient-centric programmes and psychoeducation sessions to enhance well-being and social integration
• Coordinate programme logistics including venue setup, registration, volunteer involvement, and speaker engagement
• Monitor and evaluate programme outcomes, incorporating feedback for continuous improvement

4. Volunteer Engagement & Supervision

• Serve as the main liaison for befriender volunteers, including onboarding, training and ongoing support
• Review volunteer interaction records and follow up on cases requiring intervention
• Organise and coordinate volunteer participation in programmes, outreach and centre activities
• Provide supervision to ensure safe, effective, and meaningful volunteer engagement

5. Documentation & Reporting

• Maintain accurate and timely case documentation in line with organisational and professional standards
• Prepare regular reports on programme outcomes and social service activities for management review

Woodlands Health encompasses acute and intermediate and long-term care facilities. The Medical Social Worker has an important role in supporting the Medical Social Work Department in the provision of the continuum of care services within the campus and into the community.

The Medical Social Worker will provide bio-psychosocial assessment and intervention in management of patient care throughout the various services that is provided by the campus.

Requirements

• Possess a Master/Degree/ Graduate Diploma in Social Work and for fresh graduates, to have completed minimally 400 of supervised placement hours
• Being a registered social worker (RSW) with the Singapore Association of Social Workers OR for fresh graduates, to meet the minimum requirement to be a Provisional registered social worker (P-RSW)
• Proficient in the use of Microsoft office (Word, Excel, PowerPoint)
• Ability to speak local dialects, or Malay/ Mandarin/Tamil would be strongly preferred
• Knowledge of community and medical sector and their service delivery
• Strong knowledge of social service sector and relevant Public Policies
• Strong communication skills and inter-personal skills and can manage staff across all ages
• Strong team player
• Passion in working in the health and social service sector
• Applicant with past experience or interests in providing psychosocial care in the intensive care unit and emergency department will be considered favourably.

What you will do

​The incumbent is responsible for coordinating financial assistance and liaising with various agencies on related issues such as housing and shelter, legal and ethical issues and care related issues. He or She also provides counselling to patients and families. The incumbent will be part of the multidisciplinary team to improve the quality of life for patients and families. Other tasks include participation in community programmes to create awareness of HCA’s work to the community and institutions/ agencies.

Who you are

• Degree in Medical Social Work or equivalent
• Minimum 2 years of relevant experience. More experienced candidates may be considered for the more senior appointment.
• Case management and counselling skills
• Preferably someone who is professional, a good team worker and keen learner
