We have a care management team supported by trained volunteers. Role Overview

We are seeking an experienced and dedicated Care Coordinator to oversee case management for clients with complex needs. The role involves client support, programme management, and team capacity building, while working closely with community and healthcare partners to deliver impactful outcomes.

Key Responsibilities

1. Care Coordination & Case Management

• Conduct assessments and develop care plans for clients with complex needs.
• Provide oversight and follow-up on case management and interventions.
• Facilitate referrals to appropriate community and healthcare services.

2. Partnership & Collaboration

• Build and strengthen relationships with community partners, healthcare providers, and government agencies.
• Act as a key point of contact for escalated client matters.

3. Programme Management

• Monitor service delivery to meet programme outcomes and funders’ requirements.
• Track, review and report on case progress, KPIs, and client outcomes.
• Contribute to the development and improvement of care management processes.

4. Capacity Building

• Provide training, mentoring, and on-the-job coaching for new/junior Care Coordinators.
• Share best practices and foster professional development within the team.

Requirements

• Degree or Diploma in Social Work, Nursing, Counselling, Psychology or related disciplines.
• Minimum 3–5 years of relevant working experience in case management, care coordination or social services.
• Strong leadership, team management, and mentoring abilities.
• Good assessment, problem-solving, and decision-making skills.
• Effective communication and interpersonal skills with diverse stakeholders.
• Ability to work independently and collaboratively in a fast-paced community setting.

Hiring: Receptionist cum Coordinator

Location: Orchard Campus (nearest MRT: Somerset station)

Working hours: Mon-Fri 8.30am to 6pm

Salary range: $2200-$2500 per month (commensurate with experience)

Responsibilities:

1. Answering phone calls, answer enquiry and divert the walk in guests or calls to the related personnel
2. Assist Customer Service to ensure the brochures are available
3. In charge of booking of Conference Room
4. To serve drinks to guests as and when required
5. In charge of arranging local and oversea courier services
6. In charge of arranging dispatch from campus to campus
7. In charge of marketing gifts and packing of door gift for marketing events
8. Assist the Academic Department in administrative work (such as: data-entry into IMS System, assist in timetabling matters, stamping of certificates, preparation of staff/teachers/students pass)
9. Assist the Academic Department in operational matters such as informing the teachers on new-join induction, distributing/collection of stationery/petty cash etc
10. Assist the Customer Service Department in operational matters such as updating of attendance, students leave form/records of confiscated items etc
11. Assist the student care team in managing students during dismissal / arrival etc
12. Other duties as assigned by HOD and Management
13. Execute the workplans of the department to realize the organization’s objectives
14. To comply with EduTrust policies, procedures and documentation
