Requirements

• Must hold LEW License (Grade 7 / 8 / 9)

• 2 years of experience in electrical support and able to review Single-Line-Drawings (SLD) independently

• Knowledge of AutoCAD and Microsoft Office

The Medical Social Worker is to conduct mental health assessment, provide home based direct intervention, support to clients and/or caregivers with mental health issues, and/or dementia in accordance to the Community Intervention Team (COMIT) programme requirement. For instance: psychoeducation, case management and counselling, as well as caregiver support and training.

Responsibilities and Duties:

- Conduct assessment and provide intervention and support to clients with mental health issues and/or dementia, eg. psychoeducation, case management and counselling, as well as caregiver support and training.

- Plan, implement and review services and activities to meet the needs of the clients and/or caregivers.

- Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance to the COMIT programme requirement.

- Keep and maintain client and programme data and documents, in accordance to the COMIT programme requirement, as well as the prevailing personal data protection and security guidelines.

- Support in the development of COMIT service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).

Job Requirements:

- Degree / postgraduate diploma in Social Work.

- Must have an interest to work with the adults above 40 years old and the elderly population in the community setting.

- Experienced in Community /Hospital setting for at least 2-3 years.

- Good team player and possess good interpersonal, communication, writing & IT skills.

- Familiar with the healthcare environment.

- Ability to communicate with vernacular languages speaking clients. 

-  Ability to multi-task, work independently and network with other service providers.

- Candidates with dementia or mental health training or working experience will be an added advantage. 

-  Has experience using psychotherapy with mental health patients.

Role & Responsibilities

• Assist the Head and/or Counsellor to attend to referrals for placement, conduct intake and admission interviews

• Use appropriate assessment tools to assess risks, needs, strengths of residents and work collaboratively with the Probation, Child & Adult Protective Services Officers to develop the Individualized Care Plan (ICP) with a view towards reintegration with family and with the larger community

• Conduct in depth biopsychosocial assessment of vulnerable families, design and deliver client centric interventions to vulnerable families

• Conduct regular meetings with relevant partners and stakeholders to evaluate resident’s progress

• Assist with research and with reviewing and developing processes to improve casework as well as service delivery

• Assist with presentations to stakeholders, visitors, professionals for outreach engagements

• Any other duties as directed by the Head of Centre and or Cluster Director, Family Services

Qualifications, Competencies & Experience

• Degree or Master of Soc Sc (Social Work) – for Senior Social Worker 

• Masters in Social Sciences (Professional Counselling) – for Senior Counsellor

• Minimally 3 -5 years experience or more working with at-risk youth with trauma and families
