Key responsibilities Conduct home visits to assess, diagnose, and manage acute and chronic health conditions in patients of all ages. Develop and implement evidence-based care plans, including prescribing medications, ordering diagnostic tests, and providing therapeutic interventions. Collaborate with GPs, specialist services, social care agencies, and other healthcare professionals to coordinate seamless care transitions and optimise patient outcomes. Provide patient education and support to promote self-management, healthy lifestyle behaviours, and adhere to treatment plans. Participate in multidisciplinary team meetings and clinical audits to enhance service delivery and quality improvement initiatives. Maintain accurate and up-to-date electronic health records, documentation, and clinical correspondence in accordance with regulatory standards and practice policies. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols and within scope of practice. To work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations including initiation of effective emergency care. Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principle of selfcare. Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for mental health. Carry out the partners instructions and requests. All staff are required to take their share in providing cover, when necessary, for colleagues who may be absent as a result of holidays or illness. Maintain a smart appearance and be helpful wherever possible All staff should be conversant with and adhere to all written guidelines and protocols. Extra Duties and Responsibilities To produce accurate, contemporaneous and complete records of patients consultation, consistent with legislation, policies and procedures. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. IPC compliance and waste disposal Deliver care according o NSF,NICE guidelines and evidence based care. Maintain practice effective communication within practice environment and with external stakeholders. To raise any significant events and discuss learning opportunities in clinical and MDT meetings. Target outstanding QOF and arrange for this to be completed. Liaise with colleagues and ensure hand over is carried out. All patient identifiable data is locked away at the end of the day. Organisational and Statutory Requirements The Post Holder must be aware of the responsibility placed on him/her under the Health & Safety at work Act 1974 and ensure that agreed safety procedures are carried out in order to maintain a safe environment for employees, patients and visitors.
