This is a consultant post in Infection (Medical microbiology or Medical Microbiology and infectious diseases) The service is delivered mainly for 2 acute sites, the University Hospital of North Durham and Darlington Memorial hospital. Based on service needs and in agreement with the appointing manager and applicant, the applicants base will be University Hospital of North Durham OR Darlington Memorial Hospital. The post holder will however be required to work at any location within the Trust. The appointee will be expected to be involved in implementing the Trusts Clinical Governance programme. This includes active participation in clinical audit, quality, clinical guidelines/pathways, and professional development. The post holder will provide, working with other colleagues, clinical advice on medical microbiology to clinical staff in hospitals and in the community, and within the area served by the laboratory and consultants in communicable disease control, environmental health and others. This will include participation in surveillance, hospital infection control and public health microbiology. The appointee will be expected to work with local managers and professional colleagues in the efficient running of the services and will share with Consultant colleagues in providing a clinical microbiology service to the Trust and local CCGs. This post will predominantly cover clinical service but in addition may have the need for the appointee to attend RCA, MDT meetings where required. There will be a compulsory on call commitment as part of this post Infection Control The substantive post will involve working with the Consultant colleagues and infection control nurses to deal with day to day infection control management across the Trust. Active involvement in investigation of infection control incidents and in action planning and implementation to reduce hospital acquired infection is expected. There will be a need to be involved in root cause analysis following infection control incidents. There will also be a need to be involved in decontamination issues. Laboratory To be involved in all aspects of the day-to-day operation to UKAS standards of the Clinical Microbiology laboratory with appropriate technical support. The Consultant will provide a high level of clinical expertise and leadership, both for the team for which they are responsible and in borough-wide services. CLINICAL ASSESSMENT AND CARE OF CHILDREN To provide assessment and case management of children and young people with complex Neurodisability and special needs, including in Special Needs schools in the Borough. The Consultant has overall responsibility for their own caseload, which will include children across the borough or living out of borough but registered under a Greenwich GP. To provide supervision of non-Consultant grade doctors, including Speciality Doctor Paediatricians and Specialist Trainees. Provide clinical input to multi-disciplinary clinics for the assessment of children with complex disability and special needs. These assessments include a rising caseload of children with ASD and ADHD that require assessment and further management. Provide Consultant input to medical follow up of children within the special schools. Provide medical advice to Local Authority Childrens Services according to statutory legislation for children with special educational needs requiring Education, Health & Care Plans (EHCP). The Consultant will be supported to develop and pursue their interest in Community Paediatrics.

There is a degree of flexibility within the job plan and this example is for illustrative purposes only. Where individuals have a speciality interest this can be considered and job planed if mutually beneficial.This schedule outlines the Direct Clinical Care component (DCC) of the job plan. In addition, the candidate will be on the General medial on call rota and MAU weekend rota. SPA activity and clinical admin can be delivered flexibly around the clinical shifts. SPA activity is outlined in the accompanying table and is delivered over the 5-week cycle rather than per week. It would be expected the successful candidate would organise their own timetable with regards to fitting this activity into the job plan. There would be flexibility to swap shifts to prevent longer periods of work inadvertently caused by the on call rotas. The successful candidate would not work though these weeks sequentially. Rather based on leave requirements the rota would aim to accommodate consultants shifts based on weekly availability. The shifts are part of an annualised job plan.

General Responsibilities of the Consultant General Surgeon The following is not an exhaustive or exclusive list of the duties of a Consultant Grade. The appointee is expected to: Provide senior clinician services to both the Emergency Department and the General Surgery Department as per job plan or as agreed within the Trust, and as required when on call. Work with consultant colleagues and nursing staff to ensure efficient use of Trusts resources and contribute actively to clinical governance in the department and to development of quality enhancement projects in the unit. Provide senior clinician services to area of special interest as per the job plan or as agreed within the Trust. Show flexibility and undertake different appropriate clinical tasks within allocated DCC sessions at the request of the clinical manager, as the need arises. Actively participate in the Units Teaching Programme and undertake supervision, teaching and training of medical trainees and other multi-professional staff, in line with guidance from the relevant Royal Colleges or specialty bodies. Actively participate in clinical guidelines development, audit and CPD. On-call frequency will be 1 in 12 including weekends (STC) and includes weekend ward rounds to comply with London Emergency Surgical Standards. Unpredictable work is carried out from home, returning to work within 30 minutes as dictated by the caseload. There are 10 SPAs. Thus this post represents 7.5 DCC to 2.5 SPA. Upon agreement with the Divisional Clinical Director, the post holder can take on additional SPA activities. This post has been devised to provide consistent Consultant presences on the Acute Medical Units ensuring national quality indicators for acute medicine are met. Consultant Acute Physicians will provide leadership onthe Acute Medical Unit and in ED from Monday to Friday, supporting acute teams in the assessment, initial treatment and care planning for acutely ill patients referred from ED or local General Practitioners.They will be responsible for the day to day clinical management of the unit and develop strong links with the ED, radiology, pathology and other clinical specialty teams to create efficient care pathways to improve thepatient care. Leadership of the multidisciplinary team is required to promote early discharge. The daily rapid access admission avoidance ambulatory outpatient service is also to be supported by the appointee with the existing Consultant Acute Physicians.
