The post holder will be responsible for the day-to-day management of divisional patient safety incidents in accordance with the Trust’s PSIRF Policy and Plan
The post holder will work closely with the Clinical Governance Lead, the Governance Manager and the Divisional Triumvirate to ensure that clinical governance practices and procedures, especially in relation to PSIRF, are well embedded and adhered to.
This is not an exhaustive list of duties, and a regular review will take place with the postholder as part of their ongoing development and performance management.
• responsible for daily monitoring of patient safety incidents
• responsible for working with Divisional Head of Nursing and Clinical Governance Lead to facilitate the bi-weekly divisional governance meetings. This includes organising and attending a pre-meet in order to agree incidents for escalation and agree agenda, collating and disseminating papers and reports for the meeting; and presenting and facilitating the patient safety incident aspect of the agenda.
• Responsible for creating patient safety incident reports for a wide variety of specialty meetings, boards and committees.
• Responsible for presentation of Governance aspects of mandatory training.
• Responsible for provision of training to all staff on the use of the Trust’s patient safety incident reporting system.
• Responsible for day-to-day maintenance of the Trust’s patient safety incident reporting system.
• Assist with the dissemination of learning from patient safety incidents, including follow up of actions.
• Undertake education and training relevant to role and to enable reasonable quality of role cover within the team in the absence of departmental leads.
• Ensure own mandatory training requirements are met.
