As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. You will be responsible for working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need. Responsible for care coordination of an identified caseload and you will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate. This is a 24 hour service and team members are all practitioners are required to work rostered shifts including weekends and nights and you must have a driving license and access to a car for your shifts. Please refer to the attached job description and person specification for full details of responsibilities The Return to Practice Nurse will be undertaking a training programme within Cambridgeshire and Peterborough NHS Foundation Trust and Anglia Ruskin University. A placement within an agreed clinical area will be undertaken as part of the course. A minimum of 150 hours will be spent in clinical practice. The Return to Practice Nurse will assist in the overall implementation and evaluation of care as part of the multi-disciplinary team. The Return to Practice Nurse is expected to plan, implement, deliver, and maintain high standards of care. The Return to Practice Nurse will be skilled in taking appropriate action(s) to achieve the planned outcome. The Return to Practice Nurse is expected to work under supervision across a 24-hour service within an agreed clinical environment. The Return to Practice Nurse will be expected to practice in accordance with Trust policies, guidelines, standards, and statutory requirements and operate within defined competency boundaries. It is required that Return to Practice Nurses will commit to undertake the formal education of the course and successfully complete all components of the training programme within the contract period.

To provide leadership and management to a multidisciplinary Community Nursing team, providing a multidisciplinary holistic approach, ensuring provision of high quality, effective, well-co-ordinated and responsive care to the changing needs of patients and the local population. To demonstrate effective leadership by taking an active role with the professional development of the workforce, improve standards and promote good practice and clinical effectiveness, lead and manage change through a multi-skilled team approach, supporting the re-shaping of services in line with local / national policy. This post is based at the National Hospital for Neurology and Neurosurgery (NHNN) which is the largest division within the Specialist Hospitals Board of the University College Hospital Foundation NHS Trust. This postholder will work alongside the neuromuscular clinical research team. The post holder will be required to work according to the NMC Code of Professional Conduct and relevant professional guidelines, taking responsibility for the coordination and support of clinical trials within the CNMD. Responsibilities include coordination of care of patients on trials, collection and preparation of biological samples, administration of experimental agents and monitoring of expected and unexpected side effects of drugs and other treatment modalities, and patient/carer education. The post holder will be expected to develop specialized skills and knowledge relating to the conduct of clinical trials, the therapies and relevant disease types. The post holder will be expected to have sufficient skills to enable the patients to make informed choices concerning their involvement in clinical trials by providing advice and information and acting as the patients advocate.
