The Job:

Reporting to the PHICO Manager, the Senior/Executive will support the SGH Population Health and Integrated Office (PHICO) in administrating the Healthy, Empowered and Active Living (HEAL) Fund and its projects within Southeast Singapore and the wider SingHealth Regional Health System area. The projects cover a wide range of population health areas, including care model innovation, technology-enabled care and intergenerational programmes.

The roles and responsibilities include but is not limited to:

• Support in programme and fund management, including developing work plans, report writing, budgeting and delivering presentations
• Facilitating population health projects, from development to implementation and support in scaling
• Working closely with various internal and external stakeholders across various sectors to plan and facilitate programmes activities to support population health projects under HEAL Fund

The incumbent will provide administrative support to the Assistant/Senior/ Manager/Assistant Director and Executives of the Institute in the day-to-day operations, as well as planning and implementing PGAHI’s initiatives and programmes.

General job duties:

• Provide administrative support in the Institute’s routine operations in areas such as inventory (including stock taking and purchasing), venue bookings, etc.

The Job

You will be responsible for the accurate and timely abstracting and coding of diagnoses and procedures for retrieval, research and reimbursement purposes.

Your job role will include but not be limited to the following:

• Abstracting of complete and accurate, diagnoses and procedures after a thorough review of the entire inpatient records and assign appropriate ICD-10- AM Codes
• Communicating effectively with health care professionals to clarify any incomplete or ambiguous documentation

The Job:

Reporting to the Manager, the main accountabilities of the Executive includes providing and coordinating operational and administrative support to ensure smooth and effective operations of PHICO programmes.

The roles and responsibilities of the Executive include but is not limited to the following:

• Engage and liaise with internal and external stakeholders, including clinical and operational departments from SingHealth institutions, external community partners such as eldercare service providers, primary care partners and government agencies involved in the programme
• Monitor programme progress and provide regular updates to programme director, senior management and relevant agencies/funder
• Facilitate and support regular meetings including case discussion in multi-disciplinary meetings, administrative meetings, and preparation of minutes
• Work with business units to understand data requirements and establish criteria to collect the relevant information for analysis and evaluation of programme. Ensure timely submission of indicators and reports to programme director and funder

The Job:
