As a transplant coordinator, you are able to impact the lives of our patients and their families. You are required to identify potential donors from the pool of brain death and cardiac death in SingHealth hospitals. You will be part of the multidisciplinary team alongside the patients during their transplant journeys within our transplant programs in SingHealth. You will also support the Organ Donation Oversight Committee as a transplant advocate and assist in organ donation initiatives to create awareness to internal and external stakeholders.

You will acquire clinical knowledge (medical, surgical, human physiology and organ tissue-cell specific anatomies), be familiar with common ailments and chronic diseases and be well-versed with legislations that permit organ donation. You are required to order and understand basic interpretation of diagnostics and laboratory tests. You will perform on-call duties on a rotational basis to support the organ donation and transplant programmes.

You will also be trained to deliver care to more than one transplant program to support the strategic goals of SingHealth Duke-NUS Transplant Centre (SDTC) and be responsible to provide follow-up, education and care across the continuum from pre-transplant and post- transplant in accordance to established protocols and guidelines. You will also participate in transplant-related projects carried out by SDTC.

Overview*

The Care Manager supports and works within Alexandra Hospital, and collaborates with patient’s care team, community service providers, government agencies, and multi-disciplinary hospital and healthcare teams to provide coordination and continuity of patient care across the healthcare continuum; to support patients and their family members in navigating their healthcare journey with Alexandra Hospital within the Queenstown community.

Job Responsibilities*

You will be responsible for the following:

• Attending to medical queries received via a 24/7 integrated telephone hotline that forms a network for triaging patients, and provides an avenue to caregivers and community partners to connect with Alexandra Hospital for information relating to the health and well-being of patients and clients within Queenstown community.
• Ensures that the medical query is escalated to the appropriate medical provider and follows up with proper case closure.
• Follows up on all calls after 2 weeks to review patient’s disposition.
• Has the expertise to navigate non-clinical calls appropriately
• Provides guidance and support to a team of Care Manager Associates.
• Oversees the team’s rostering schedule and claims administration for shift duties.
• Performs morning and afternoon shift duties as rostered, and be the key person to standby during manpower shortage during night shifts.
• Support VCC expansion plans and attend meetings with internal and external stakeholders.
• Participate in activities that contribute towards the improvement of patient care, including professional development sessions to develop relevant areas of knowledge, skills and attitudes.
• Participate in projects and/or community events organized by Alexandra Hospital or partners within Queenstown community.
• Any other duties as assigned by Reporting Officer.

Requirements*

• Degree or equivalent professional qualificationsin Nursing, Social Work or Allied Health profession.
• 3 – 5 years of experience in healthcare settings is preferred.
• Knowledge in geriatric and community care will be an advantage.
• Ability to perform 24/7 shift work is required for this role.

You will perform clinical screening and assess suitability of referrals for tissue donation. You will provide support to the tissue recovery team through collection and interpretation of background information, including medical histories and other relevant information on deceased persons. You will liaise with families regarding tissue donation and ensure that the families are well supported throughout the process.

You will be collaborating closely with relevant stakeholders to ensure the timely acquisition of necessary authorisations and consent mandated by legal requirements and ensure strict compliance with required standards throughout the recovery process. You will ensure the distribution of tissues are carried out in accordance with the guidelines by each programme/MOH. You will update the registry for every referral case, collate and disseminate statistical data for the tissue bank as required.

You will participate in activities and outreach efforts aimed at raising awareness about tissue donation among the general public and be involved in transplant tissue bank education for healthcare professionals, students (medical, nursing, and allied health) and overseas visitors. You will also assist with maintaining the Standard Operating Procedures and guidelines that comply with established legal frameworks and accreditation bodies, as well as participate in research related to tissue transplantation.

Requirements:

• Bachelor’s Degree in Psychology, Social Work, Nursing, Healthcare, Life Sciences or equivalent, preferably with 1 to 2 years of working experience. Fresh graduates are welcome to apply.

Your Profile / Qualifications

Professional Requirements

• Candidate with experience in the engineering service industry
• Proven experience in service sales/co-ordination and order processing; experience as a team leader or other similar previous responsibilities will be considered a plus.
• Candidate with diploma in engineering is preferred.
• Experience in marine industry is preferred
• Knowledge and experience with SAP
• Good computer skills with a sound knowledge of the following software packages, Microsoft Office Products (Excel, Access, Word, PowerPoint).
• Well-organized and responsible with an aptitude in problem-solving

The role of an Advance Care Planning (ACP) Programme Coordinator involves facilitating appropriately staged ACP discussions with patient and/or their family members, to document decisions reflective of patient’s wishes and plans for future care. 1/ Coordination Tasks

• Handling of all incoming service requests and spare parts sales processing
• Reviewing e-mails and prioritising responses taking into account time scales, time differences, shipment of spares, urgency, commitments to customers and outside assistance (Suppliers, Tech Support, Purchasing, Sales and Finance)
• Liaising with ships agents worldwide to arrange the attendance and shipment of parts.
• Responsible for service confirmation, quotations of worldwide services rates to customers including expenses and checking availability of spares.

• Responsible for agreeing scope of work and charge point with customer. Ensuring the relevant purchase order is received if necessary.
