Clinical The reception, diagnosis and emergency treatment of patients presenting to the adult Emergency Department. The successful candidate will be given supervision and support from senior medical staff to develop competency in the autonomous clinical management of these patients whilst ensuring patient safety. This post is based in the adult Emergency Department and no sessions will be undertaken in the paediatric Emergency Department. There is close liaison between ED and ICU at BRI. Patients referred to Specialist Units within the hospital are the responsibility of that specialist team, who will see the patient in the Emergency Department and arrange appropriate care. However, medical and nursing staff in the ED are responsible for any emergency care necessary. This shared care is seamless and collaborative. Managerial The management responsibility of the post holder will be to the Clinical Director who is responsible to the Chief Executive and Trust Board. Clinical Audit and Clinical Governance The post holder must be aware of clinical governance and clinical risk management and take an active part in their implementation, including audit. Clinical audit is established at the BRI. They will participate in at least one supervised audit project during the 12 month post and will be encouraged to attend interdisciplinary clinical governance meetings On-Call Commitment In ED the roster is a full shift system. Shifts are a balanced mix of earlies, mids, lates, and nights. The posts pay will be calculated by HR and will be compliant with the new regulations set in Aug 2017.

The working hours for this post will be Monday Friday, 8.30am 16:30 although we will expect the post holder to work with their team flexibly in deciding the exact working pattern that best suits the post-holder and team. There is no on-call requirement for this post. The post holder: Is responsible to the Consultants and Registrar for the execution of their professional duties. They shall ensure that all patients admitted to the department are medically examined and assure that robust and appropriate treatment plans. They will attend ward rounds as arranged with Senior staff to acquaint themselves with patient management, and also at other times daily to ensure that all investigations, treatments ordered are carried out. They will ensure that good communication is maintained with staff in all specialties to the benefit of the patient. The will work as a valued member of the multi-disciplinary team and contribute to meetings and team decisions on patient care. They are responsible for the daily maintenance of in-patient notes, preparing Discharge letters (TTAs) for immediate dispatch to the patients General Practitioner and communicate with primary care and community services as needed. They shall make themselves available to talk to patients relatives if required, discuss treatment escalation plans and advance care planning decisions as needed. In carrying out their duties the Doctor will always be responsible for keeping their Senior Staff informed of serious cases on the ward, deaths, and any other matter which it is deemed necessary that their attention should be drawn to. Any cases in which there is doubt must be referred to a senior member of staff. To work as part of a team in the provision of an efficient and smooth running clinical chemistry service. The post holder will be required to perform a range of diagnostic tests and support functions across all sites within the Trust. Clinical Attend and assist in the out-patient clinics, taking responsibility to ensure adequate staffing for the clinic and to facilitate its efficient running in any other way. Clinic letters must be dictated promptly. Attend consultant ward rounds. Make formal ward rounds daily including at weekends and holidays as part of the on-call responsibilities. These rounds should be considered a teaching opportunity for more junior staff. Participation in the relevant rota will depend on clinical experience of the successful candidate. Out of hours responsibilities include: Night-time supervision of care of surgical patients including those in the Intensive Care Unit/Recovery Unit and communicate appropriately with the responsible consultant regarding major problems arising with that consultants patients. Weekend on-call responsibilities include ward rounds as well as participation in emergency operations and assessment of in-patients or outside referrals. Advise on the discharge of patients under the care of the cardiothoracic surgical consultants. Theatre responsibilities Ensure that the theatre cases are appropriately staffed. Ensuring that junior staff are involved at an appropriate level wherever possible. Junior staff must be assigned to cases whenever ward responsibilities permits. Ensuring that adequate junior level assistants are available to help open and close cases performed under the direction of, or with the assistance of, a consultant. Ensuring that all cases are sent for promptly and facilitating the efficient transfer of patients if required. Ensuring that communication is maintained with the relatives of patients. Ensure that either the operating surgeon or senior level assistant involved in the operation complete all appropriate data entry tasks at the conclusion of the operation. In addition, the principal operator should be responsible for creating a suitable operation note, which accurately records the events of the procedure. If operations are delayed or cancelled, the post holders are responsible for communicating with the wards appropriately and explaining the situation to the patients. Admission responsibilities Communicate with the consultant staff regarding their respective theatre schedules individually and co-ordinate hospital admission in an integrated and coherent manner such that hospital beds are utilised most efficiently. This may require suggesting modifications to the planned admissions schedule to individual consultants on the basis of in-patient referrals, emergency cases or theatre cancellations. Participation in weekly planning meetings is required. Waiting list management Maintain knowledge of the active waiting list for your consultant and participate in their selection for admission and surgery in a manner that is equitable for patients as much as possible. Be involved in the evaluation of patients referred to the service, particularly in-patient referrals. Respond to queries from the patients and their GPs (both by telephone and letter) regarding their position on the waiting list and anticipated time of operation. Administration of service As part of their overall responsibilities for the patients, post holders must ensure that medical records are adequately kept by the junior medical staff including daily notes on in-patients and prompt dictation of discharge summaries. Notes must be representative and appropriate to the patients true condition. Medical audit Post holders must ensure that all junior staff are present at the monthly medical audit meeting. Opportunities to present cases for discussion from the firm will arise and should be prepared for and presented by post holders or assigned to a colleague for presentation. Research Research is an integral component of the training programme. Post holders are encouraged and expected to participate in clinical or basic science research. Teaching Post holders are expected to take an active role in the postgraduate teaching activities. In addition he/she may be required to undertake teaching of medical students, nurses and other paramedical staff. ORTHOPAEDIC JUNIOR CLINICAL FELLOW (CT LEVEL) – CT level available for up to 12/24 months commencing within six months of the interview date at the latest, the role comprises a full time commitment plus out of hours working.

This position will offer suitable candidates full educational support; Educational Supervisor, funded study leave and paid courses as appropriate. Funding of up to £5,000 per annum will also be available to assist with continued orthopaedic training and qualifications.
