We have the exciting opportunity for Nurse Support Practitioners to come and join our new Complex Transition Support Service! This is a permanent, full-time role, Monday - Friday. The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation. About the job As a CTSS Nurse Support Practitioner, you will work with service users and relevant external staff teams to ensure timely, safe and effective transition from acute inpatient areas into community settings.
