Senior Registered Nurse - specialising in ABI and other neurological conditions.

Our client provides specialist nursing care and rehabilitation services for people who are living with an acquired brain injury (ABI) and other neurological conditions, who need support and assistance, or the ability to rebuild key life skills so they can lead fulfilling and rewarding lives.

Ideally the candidate would have working experience as a senior nurse, leading and planning shifts, coordinating MDT and GP rounds and providing clinical oversights as well as audits and reports.

The post holder will support the development and implementation of the Healthy Child Programme with a particular focus on the high impact areas of breastfeeding, infant nutrition, healthy weight and healthy nutrition.

Specialist Community Public Health Nurse (Health Visitor)

To plan, organise and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high quality service.

To provide a skilled, effective and evidence based health visiting service to clients in a variety of community settings.

To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.

To collaborate with team colleagues to deliver the Quality Strategy objectives.

To collaborate and deliver an integrated service delivery in partnership with the Family Hubs and our partners which will improve access and health outcomes for children and their families.

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C’s apply)

Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required

Utilising specialist skills and knowledge, advise and support individuals and families on evidence based health care practices

Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs

Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children

Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population

To develop and delegate appropriately the implementation of regular health promotion initiatives on specific health issues and to specific groups and individuals
